Mission: To provide exceptional healthcare to people facing access barriers through clinical programs and education, serving over 92,000 people living in the pikes peak and east central plains regions of colorado through 26 health centers spread across 20 service locations. Peak vista offers numerous ancillary services and programs to benefit our patients to include: first visitor which provides information to parents before and after their baby is born with trained volunteers making home visits designed to connect parents with community resources and early childhood materials. Health education supports patients with management of chronic diseases such as diabetes, high blood pressure, obesity, and asthma. The three pharmacy locations provide easy access to counseling and medications. Well child waiting areas welcome healthy children into a secure and child-friendly environment so that patients can attend clinical appointments without the worry of finding outside child care.
Programs: Primary care: peak vista served 87,832 medical patients with 285,224 visits in 2017. Additionally, through our primary care program, peak vista provided care coordination and support services for 74,194 visits, 18,938 patient visits to our convenient care center, 29,790 visits associated with our behavioral health services, and 1,123 visits through our volunteer specialty care center. dental: peak vista served 14,810 dental patients in 2017 with 35,984 visits. Dental visits occur in family health centers and our main dental center. Dental hygienist services are also integrated into our pediatric health centers.
homeless: the homeless program provides primary medical, dental care, and behavioral health care. Peak vista served 1,621 patients though 5,239 visits in 2017. Our homeless services are located adjacent to community partners to better serve the homeless population.
