Mission: Provide hiv supportive services in a safe environment designed to encourage and empower those affected to become self-sufficient members of the community. All services are provided free of charge.
Programs: Client support - approximately 1,750 units of housing case management and supervision was provided to approximately 200 clients. Client supportive services including mental health and substance abuse counseling, nutritional guidance, and food and food services(9,320 meals) were provided and assistance with transportation was provided to 180 clients. Emergency assistance - direct rent and utility assistance for individuals and families infected, affected and at risk for hiv/aids. Approximately 25 clients received rent and utility assistance. Additionally, food and transportation is provided to eligible participants.
medical case management - case management linking clients with health care, psycho social, and other services to ensure timely and coordinated access to medically appropriate levels of health and support services. Approximately 7,970 units of case management were provided to 740 clients.
