Mission: To provide nurse care coordination for children who are licensed in foster care of under dcf protective services in sarasota, manatee and desoto counties.
Programs: Ymca foster care program - holds a contract with the sarasota ymca to provide nurse care management for children in foster care that are healthy and do not qualify for medical foster care. These children are unable to live with their families and have been placed in the custody of the department of children and families. These children are birth throught 21 years old, have medicaid funding, do not qualify for medical foster care and are placed with a licensed foster family in sarasota, manatee and desoto counties.
medical foster care program - holds a contract with the state of florida to provide medical foster care for children that qualify. Medical foster care is a home-based program for the children that are medically involved and who are unable to live with their families. This program offers many services to the medical foster care parents, the children and their birth families. The children that qualify are those that have been removed from their birth family or guardian and placed with the department of children and families, are birth through 21 years old, have medicaid funding and have a special medical need. These children are dual enrolled with the watch primary care program and childrens medical services. Children served are in sarasota, manatee and charlotte counties. These children are first staffed through a children's multidisciplinary assessment team to see if they qualify.
