Medical foster care is a home-based program for the children that are medically involved and who are unable to live with their families. This program offers many services to the medical foster care parents, the children and their birth families. The children that qualify are those that have been removed from their birth family or guardian and placed with the department of children and families, are birth through 21 years old, have medicaid funding and have a special medical need. These children are dual enrolled with the watch primary care program and childrens medical services. Children served are in sarasota, manatee and charlotte counties. These children are first staffed through a children's multidisciplinary assessment team to see if they qualify.
