Programs: The unfunded pilot program began in april 2015. It was created in an effort to decrease avoidable hospital utilization among unfunded patients in our community. High utilization unfunded patients entered the program through referrals from the chi st. Joseph ed and case management departments. Those referrals were screened to evaluate patient medical history, utilization history, and funding status. Financial assistance provided to these patients included medications, dme, clinic visit payments, transportation assistance for medical appointments, and assistance with cell phone minutes to ensure the ability to provide telephonic care coordination to the population. A rn care coordinator helps collaborate among several community providers for additional needs beyond what the program was designed to assist with (texas a&m navigators, st. Vincent de paul, etc. ). The care coordinator provides care coordination and disease management education to these patient's on an outpatient basis in addition to assisting with certain financial needs related to their healthcare. The decision to discharge a patient from the program was based on several factors (i. E. Obtained funding/needs met, noncompliance, unable to reach, etc. ). The program includes an average of 9 patients.
