Mission: Assist individuals with intellectual and developmental disabilities by building supportive partnerships and connecting them to resources that enable them to live the life they choose.
Programs: Community coordination - helps individuals with disabilities who are authorized for services by the state to identify, choose and attain their preferred lifestyle, as participating members of their communities, with equal rights and responsibilities. The resource coordinator helps to locate and develop necessary supports and services, assuring that each individual's needs and preferences are addressed, and then monitors the delivery of services to ensure efficacy and individual satisfaction. This program served 2,018 individuals during the current fiscal year.
comprehensive assessment - assists individuals with completing the developmental disabilities adminstration (dda) application process and provide referral for community resources based upon assessment of needs. The program processed 81 applications in the current fiscal year.
waiting list coordination services - assists individuals who are on the developmental disabilities administration's (dda) waiting list for services. Case managers connect individuals with resources in their communities in order to stabilize crises and to promote independence and community participation. The program served 456 individuals during the current fiscal year.
