Mission: Mchc's mission is to deliver high quality primary and specialty medical, dental, rehabilitative and behavioral health services to patients of all ages and abilities, including those with chronic health conditions, in culturally sensitive environments, without discrimination or regard to ability to pay. The national committee for quality assurance has designated mchc as a level 3 patient-centered medical home. Mchc was initially incorporated by united cerebral palsy associations of nys, inc. And became an independently governed and managed fqhc network in august of 2015.
Programs: To operate a network of diagnostic and treatment centers to provide, or arrange for the provision of, primary and preventive quality health care including related ancillary and enabling services for all residents of the organization's service area within the state of new york.
