Mission: The liberty rehabilitation center faculty practice plan (the "plan") was established to provide medically necessary inpatient professional rehabilitative health services to the medically underserved communities of hudson county, new jersey. Patients are seen regardless of their race, color, creed, sex, national origin, religion or ability to pay for the professional services rendered. Plan physicians staff a 25-bed rehabilitative care unit. Patients seen by plan physicians are typically recovering from illnesses such as strokes, fractures, total joint replacements, lower extremity amputations, severe arthritis, neuromuscular problems (such as parkinson's and multiple sclerosis), or multiple trauma. The plan physicians direct a hospital multidisciplinary care team that includes therapists, social workers, psychologists, and dieticians.
