Causes: Community Improvement & Capacity Building
Mission: Healthteamworks (organization, a national nonprofit organization, is focused on realizing the vision of a healthcare delivery system comprised of patient-centered communities of care. Leveraging deep clinical experience, evidence-based medicine and advanced quality improvement methodologies, the organization provides innovative solutions for healthcare providers and other stakeholders. Through these solutions, improves quality of care, the patient experience, provider satisfaction, and lowers the total cost of care.
Programs: Healthteamworks is a nonprofit, multi-stakeholder collaborative working to redesign the healthcare delivery system and promote integrated communities of care. Our goals are to optimize health, improve quality and safety, reduce costs, and improve the care experience for patients and their healthcare teams. We drive measurable improvements in healthcare delivery through deep client collaboration and engagement and partner with practices, physician groups, integrated clinical networks and payers to provide the services, tools and training to achieve the quadruple aim and succeed in a value-based environment. We currently deliver practice transformation services to over 200 practices in colorado and 3,000 practices across the nation through state and federally-funded programs including cpc+ (comprehensive primary care initiative), tcpi (transforming clinical practice initiative), ensw (evidence now southwest), colorado sim (state innovation model) and delaware sim. We also provide innovative workforce development training to prepare healthcare professionals to succeed in the advanced primary care arena.
