Programs: Nursing facility initiative to reduce avoidable hospitalizations (cms nf): this project is supported by a federal cooperative agreement with centers for medicare & medicaid services (cms) with the goal of reducing avoidable hospitalizations for long stay nursing facility residents. The foundation is partnering with other new york organizations and serving as one of six organizations across the country to achieve this goal, in collaboration with a group of new york nursing facilities. new york state partnership for patients (cms pfp): this project is supported by a federal contract received from cms with the goal of reducing hospital readmissions, preventing hospital acquired conditions, and achieving other quality health care goals. The foundation is partnering with other new york organizations and serving as one of various contractors across the country to achieve these goals, in collaboration with new york hospitals. health workforce retraining initiative (hwri): this initiative is supported by the new york state department of health and its purpose is to provide multiple training projects for hospital and nursing home members staff in the areas of quality improvement, cultural competency, infection prevention, safe patient handling, and care coordination. The grant ended as of june 30, 2016, and a new two-year contract was awarded effective january 1, 2017.
