Mission: The mission of dayonenetwork is to find solutions for children and adults with developmental delays and/or disabilities in kane and kendall counties and hanover township of cook county and to make the connections for necessary services and supports.
Programs: Child and family connections (cfc) services children 0-3 years of age who are experiencing developmental delays and disabilities who reside in kane or kendall counties, over 2,000 children were served throughout the year.
community access services comprises individual services and support advocacy (issa), pre-admission screening, case coordination, and bogard service coordination. Services provided include screening to determine service eligibility and case coordination which provides system point of entry services inclding resource information, crisis response, individual service planning and service network and linkage. Issa and bogard provide individual service coordination and advocacy for specific populations. Over 1,000 children and adults were served this year.
