Mission: Central virginia alliance for community living, inc. (cvacl) envisions a community where all individuals, regardless of age or disability, maintain independence, dignity, equal rights, and a meaningful life based on their own personal choices.
Programs: Senior supportive services program:we provided 7,875 information contacts and referrals and 324 hours of care coordination services to seniors in order for them to locate appropriate services to ensure they receive the assistance needed to remain in their own homes. We provided elder abuse prevention services,including advocacy and ombudsman activities, which encompassed in-service training at long-term care facilities and long-term care facility mediation services for the elderly and their families. On 16,204 separate occasions, we transported seniors, who had no other means of transportation, to and from doctors, dentists, medical treatment facilities, banks, grocers, and other destinations, including our sites for participation in our meals program and various planned activities. Our site participants received disease prevention counseling and health screenings. In addition, seniors participated in 8,538 hours of recreation and social activities at our various sites. Along with 2,073 hours of volunteer opportunities, services we provided included service on local long-term committees and presentations regarding the services we provide. Utilizing a local legal aid organization, we provided 48 hours of legal services to seniors who were not otherwise able to afford such services. The alliance provided 1,518 contacts checking individuals and 2,921 hours for personal care.
congregate and home delivered nutritious meals program:during the current year, individuals were provided with 88,751 nutritionally balanced meals. All meals met one-third of the recommended dietary allowance (rda). Meals were home delivered or provided at a nutrition education site. Individuals attending a nutrition education site were provided transportation, health and nutrition education, health screenings and recreation.
weatherization and low-income housing energy assistance program:this program is designed to assist seniors in making their site-built or mobile homes more energy efficient, thereby reducing their heating and cooling costs, and enabling them to live more comfortably. Assistance provided included insulation of homes; repair or replacement of furnaces; and sealing ductwork, windows, and doors. During the current year, 35 homes were weatherized and/or repaired through this program.
other programs:1. Through the virginia insurance counseling and assistance program (vicap), seniors were provided with counseling on how to make informed decisions on medicare, medicare supplemental, long-term care insurance and fraud prevention. 1,515 such consultations were provided during the agency's current year. 2. Through its bedford ride program, seniors and disabled and low-income individuals residing in the city and county of bedford, virginia were provided with transportation to non-emergency medical appointments. 4,998 one-way trips were provided by volunteer drivers under this program. 3. The agency conducts other transportation programs, one of which provided 1,581 one-way trips for disabled individuals to various appointments, shopping locations, training sessions, etc. Another program provided 2,928 one-way trips to special events for our site clients. 4. During summer months, the agency helped numerous seniors by providing them with fans for their homes or, if they have medical conditions, air conditioners. 5. One of the special events the agency conducts is an annual specially-themed senior dance. Refreshments are served and door-prizes awarded to those in attendance. 6. Care transitions is a program completed in conjunction with hospital and local health care facilities to reduce medicare readmissions which are slated by medicare for readmission penalties. The agency served 214 individuals for a readmission rate averaging 3. 30%. 7. Options counseling is a person-centered program to assist individuals who request long-term support and plan for the future regarding long-term support. The agency provided 1,205 hours of options counseling. 8. Chronic disease self-management is a program to teach individuals to self-manage chronic disease states, which is accomplished through a six week program involving self-management. The agency served 36 individuals during the current year.
