Mission: Amboy medical practice is a 501(c)(3) not-for-profit corporation through which employed physicians provide primary care, specialty care, and behavioral health services in a variety of settings including clinics and office based facilities. Amboy medical practice administers these community based sites to support increased access to and care coordination of healthcare services in an effort to affect position outcomes for its patients and the population it serves.
Programs: Amboy medical practice serves over 50,000 adults, children and adolescents with primary care services and over 20 specialties including mental health professional services. Amboy medical practice provides a patient centered medical home model of care at its ambulatory locations. This team based model of care links patients with a clinical group headed by a physician that emphasizes access to care and coordination across specialties. Amboy medical practice utilizes an electronic medical records system that assures accuracy and safety of patient data. Amboy medical practice accepts most insurances, including medicaid and medicare. Continued on schedule oamboy medical practice opened two primary care centers in 2016 which provide expanded hours, same day appointments, sub-specialty care and enhanced services such as radiology. These new centers reflect a commitment to ensure preventative care is accessible to the communities served. Amboy medical center is also a member of richmond quality, llc, an accountable care organization (aco) through which it advances a number of quality metrics. In addition, amboy medical complies with meaningful use, hedis measures and other quality programs designed to provide evidence based care. As a member of the staten island performing partner system which administers the delivery system reform incentive payment (dsrip) program, amboy medical practice addresses population health measures that focus on improving the quality of care and overall health of its medicaid beneficiaries.
