Mission: We provide quality skilled home health, hospice, and personal care services to people in their homes promoting self-sufficiency and independence regardless of their financial status. Uncompensated care provided in 2017 measured at established rates was in excess of $1,090,000.
Programs: Home health program provides medical social work, home health aide, therapies and nursing services under federally sponsored medicare program and private insurance companies. Home health program provided more than 16,000 visits to over 850 clients during 2017. Service area includes hamilton, butler, clermont, and warren counties in ohio.
personal services program provides a wide range of services including personal care, meal preparation, grocery shopping, laundry, light housekeeping, errands and relief to families. Personal services program also provides private duty, a fee for service program, that offers clients a full range of home care services, including light housekeeping, chore, custodial care and skilled services. These private duty services can be provided in a client's home, in a hospital setting or nursing home. Personal service program provided more than 45,000 hours of service to more than 650 clients in 2017. Service area includes hamilton, butler, clermont and warren counties in ohio and boone, kenton, campbell, carroll, gallatin, grant, owen and pendleton counties in kentucky.
the hospice program provides compassionate end of life care, meeting the medical, emotional, and spiritual needs of individuals and their families.
