Mission: We, trinity home health services and trinity health, serve together in the spirit of the gospel as a compassionate and transforming healing presence within our communities. Cranbrook hospice care is a member of trinity home health services and trinity health.
Programs: Since 1997, cranbrook hospice care has been bringing comfort and care to people at the end of life and offers support and understanding to their families, serving greater southeastern michigan. Hospice care helps people with life-limiting illnesses remain pain-free, nourished by the support of their loved ones. Each one of our patients is cared for by an interdisciplinary team of hospice professionals, ranging from hospice physicians and nurse practitioners to social workers and spiritual care. Together, these team members collaborate to provide the best possible hospice care. In fiscal year 2017, cranbrook hospice care provided 14,155 days of patient care.
