Programs: Assessment-respite program provides 64 beds for men and women who are homeless and referred by area hospitals who need a place to recover from injury and illness for short term periods. Residents also receive access to healthcare and social services. 93% of the residents complete their medical recovery plans. 81% of the residents from the program were successfully discharged to appropriate housing and treatment. 33% of the residents from the program were successfully discharged to permanent housing. 305 individuals served
housing services provides housing advocacy services to residents which include seeking and making referrals to affordable subsidized and market-rate permanent housing, and assisting residents moving into permanent housing with the first month's rent, security deposit, and household supplies. The health and housing outreach team staff make home visits and provide aftercare services to individuals who continue to suffer with physical illnesses, mental health or substance abuse recovery needs and require more intensive assistance to maintain and sustain permanent housing.
