Mission: Axis health system will make a meaningful difference in the health of southwest colorado residents by integrating all aspects of healthcare and treating the whole person.
Programs: Behavioral health outpatient services - mental health treatment services are designed and delivered in a model that is short-term, problem focused and incorporates the recovery perspective. Traditional mental health services designed for treating depression, obsessions, compulsions, trauma, grief and life stress are available. In addition, specialty expertise for the treatment of serious, emotional disturbances of childhood and serious and persistent mental illness in adults; including schizophrenia, bipolar disorder and major depression. Evidenced based practices and promising practices are utilized to ensure maximum effectiveness of the care delivery and include: dialectical behavioral therapy, parent child interaction therapies and integrated dual diagnosis therapy. Psychiatric services including medication management are also provided. Service delivery modalities include individual, family and group therapy, as well as case management services designed to support both mental health and substance use treatment, providing a full continuum of behavioral health care. A total of 50,712 behavioral health outpatient service hours were provided in fiscal year 2017.
integrated primary care - integrated care services treating behavioral health and physical health needs are available for axis health system (ahs) patients at the la plata integrated healthcare clinic, cortez integrated healthcare clinic, and two school-based health centers that ahs operates in the region. Behavioral health services are integrated into the traditional primary care services through a care team approach that uses population-based screenings for new patients when enrolling in the clinics for services. Oral health services are part of the integrated primary care and are offered at la plata integrated healthcare clinic and axis health system oral health clinic, which opened in june 2016. A total of 13,688 integrated service hours were provided to patients in fiscal year 2017.
acute treatment unit (atu) - a 15 bed residential acute treatment unit, providing short-term psychiatric care and stabilization in a therapeutically designed and professionally staffed environment. The atu service delivery covers crisis management and stabilization services, at an intensive level that are not appropriate on an outpatient basis. It is the only psychiatric residential resource available in the entire southwest region of colorado. The atu provided 1,102 bed days to 204 residents of the region in fiscal year 2017.
other - other program service is largely the detox. Ahs operates a 24/7 social detox unit as a compliment and entry point for other outpatient substance use disorder treatment. The detox allows for short-term monitoring of patients who are withdrawing from alcohol and/or drug intoxication or dependency. Referrals are made for outpatient substance services and out-of-region residential treatment, as appropriate. Court mandated drug and alcohol monitoring services are also available to the community through this facility. The detox also can serve as a holding facility for patients on an emergency commitment (up to 5 days) with a discharge referral to voluntary treatment on a limited involuntary commitment basis, for patients awaiting court ordered, long-term treatment. In fiscal year 2017, the detox facility provided 3,516 bed days of service to 1,710 patients.
