Mission: Sixteenth street community health centers ensures access to superior quality patient centered health care to milwaukee and surrounding communities and continues to expand access for underserved populations. Our organization strives to improve the medical, mental health and overall well-being of all members of the community through counseling, support, advocacy and education free from linguistic, cultural and economic barriers.
Programs: Community health - sixteenth street community health centers provide primary medical care for all ages, with mostly bilingual board certified provider teams including internists, family practice, pediatrics, and women's health. In calendar year 2016, 37,821 patients received care at 124,323 appointments. Also, we are committed to being the leader in behavioral health in our community providing a full range of outpatient mental health services with licensed psychiatrists, psychologists, family therapists and counselors. These providers help adults with stress, grief, anxiety and panic, and children and adolescents with hyperactivity and conduct or mood disorders. Many of the provider staff are bilingual in spanish and english. In 2016 calendar year, 3,940 patients were seen at 23,479 individual appointments.
