Mission: The mission of the organization is to be people driven, committed and progressive in our relationship to individuals with disabilities and their families. The organization assists individuals with intellectual disabilities by locating, coordinating and monitoring services to meet the individual's needs. The organization's skilled case workers, known as supports coordinators work with the individuals and their families to develop life plans that encompass living and working arrangements, ongoing service needs and any other resources that will help the individual lead healthy and fulfilling lives. Supports coordinators then continue to monitor each individual's progress to ensure that services and accommodations are effective and appropriate, that individual's are integrated into their families and their communities, and individual plans are modified to reflect changes in each individual's needs or circumstances.
Programs: The organization provided services to 3,044 individuals with intellectual disabilities which totaled 77,989 hours of service.
