Mission: It is our mission to provide exceptional skilled nursing and rehabilitative care in a safe, peace-filled, spiritually-centered environment to the older adults entrusted to us. We fully commit ourselves to a circle of community with our residents as the center, surrounded by staff, families and friends; the recognition of each resident as an autonomous individual with special and unique needs, interests and preferences; the involvement of all staff in the daily lives of our residents; creating a care environment where all the members of our community work in harmony to provide a happy home.
Programs: Our snf offers short term rehabilitation, long term care, subacute services, respite care, palliative and in-patient hospice to the older adults of our community. Our care is person-centered, focusing on all the needs of each individual physical, spiritual, psychological and social. We had 431 admissions to providence rest in 2016. We discharged 423 residents back to their homes. We rendered 68,997 days of resident care, and 57 days of respite care. Providence continued to extend the boundaries of the home to the community at large, sharing expertise, programs and resources through internship programs, community information programs, health fair participation and other outreach efforts.
our adult day health care program is a community-based medical model. We provide nursing, medical, nutritional and recreational services to our registrants in a social day care setting. Our goal is to enable chronically ill, mentally and/or physically disabled adults to remain in the community through team support and to maintain their optimal level of functioning. Our average length of stay was 4. 3 years for those registrants participating in the program in 2016. We rendered 7,662 days of care in 2016. We admitted 12 registrants to the program. We had 14 discharges.
our outpatient program provides physical and occupational therapy services to the adults of our community, specializing in rehabilitation for the senior population. The program also provides continuity for our discharged short term rehab clients who choose to remain with our therapists for their rehab once they are home. We rendered therapies in 220 patient visits during 2016.
