Mission: The mission of pathways of southwestern pennsylvania, inc. Is to advance the independence, productivity, and full citizenship of individuals with physical and/or intellectual disabilities through our commitment to the principles of independence, inclusion, and self-determination.
Programs: Early intervention - the program services children from birth through two years and is designed to minimize each child's developmental delays and maximize his/her potential through specific programing. Services and supports include speech, physical, occupational, nutrition, vision, hearing, and special instruction therapies to assist families with infants and toddlers, serving washington, greene, fayette, and westmoreland counties.
community residential home - the scope of services the program offers revolves around the concept of deinstitutionalization and normalization experiences for intellectually disabled men and women. The purpose is to give these individuals the opportunity to develop the skills needed to remain in the community, serving washington, greene, and westmoreland counties.
agency with choice - this programs serves individuals ages three and up with intellectual disabilities. Individuals and/or their representative self-direct their services leading to greater choice and control over where, when, and how support services are delivered, serving washington, greene, fayette, and westmoreland counties. Services may include, but are not limited to, home and community habilitation, respite, companion, homemaker/chore, and supported employment.
adult training program- individualized day service whose goal is to enable adults with physical and/or intellectual disabilities to live as independently as possible. This program services individuals with intellectual disabilities between the ages of 18 and 60 in washington and greene counties. Older adult daily living center- individualized day service teaching and training program whose goal is to enable adults with physical and/or intellectual disabilities age 60 and over to have a well-rounded, enjoyable, and supported retirement life, serving washington county. Exceptional choices (family support services) - home-based residential habilitation, respite, personal supports, homemaker, and chore services for children and adults with physical and/or intellectual disabilities, age 3 and older, serving washington and greene counties. In-home services- in-home, non-medical personal assistance, personal care, mobility assistance, respite, companionship, and community integration support services for children and adults with physical disabilities, serving washington, greene, and fayette counties. Vendor- indirect service to assist individuals with physical and/or intellectual disabilities with payment for services to small unlicensed providers and vendors of specialized supplies, vehicle accessibility adaptations, home accessibility adaptations, respite camps, and transportation services, serving washington, greene, fayette, and westmoreland counties. Pennsylvania elks home service program- a medical care coordination service by registered nurses for individuals with intellectual disabilities, serving southwestern pennsylvania.
