Programs: Hospice of health first, inc. Provides comfort care to the terminally ill and their families by furnishing pain control and symptom management across the health care continuum (hospital, home, nursing home, & alf) as well as educational and bereavement programs to enhance the coping abilities of families. In fiscal year 2017, hospice of health first, inc. Provided 78,824 days of care. Patients and families were provided a total of 6,834 continuous care hours in the home and 1,519 un-billable hours of continuous care. Volunteers gave a total of 12,378 hours of service to our patients and families. Hospice of health first, inc. Includes a field program, the william childs hospice house and the cape canaveral hospital inpatient unit. Bereavement services were offered to 1,328 surviving care givers. This included support groups, social groups, and the bright star center for grieving children and their families. The bright star center welcomed an additional fulltime msw and added new programs for infant and stillborn loss, adult craft group, family game night and other interactive programs benefitting our community. Major program accomplishments include: 1. Hospice of health has collaborated with a pharmacy pbm to increase efficiencies with formulary and spend, as well as cr 8358 reporting. 2. The william childs hospice house implemented a pyxis medication management system to allow first does medications to be available to the patients on arrival, decreasing the time it takes to transfer a patient from another location. 3. Health first medical group in conjunction with hospice of health first hired a fulltime palliative care medical director for inpatient consults at the tertiary hospital. 4. Hospice of health first hired a part time arnp to begin a community based palliative care program5. Our new administrative offices- combing our adult and children's grief programs have lead to increased visibility in the community and enhanced staff satisfaction.
