Mission: Horizon Hospice has provided palliative and hospice care to terminally ill people since 1978. Four volunteers established Horizon Hospice and this tradition continues with over 3,000 hours of volunteer services provided in 2003. Horizon is an urban hospice, providing care to Chicago metropolitan area residents comprising all economic levels, cultures, races, and religions. Horizon serves patients regardless of their ability to pay and has expanded outreach to many of the traditionally underserved areas on the city?s south and west sides. To further its mission to improve end-of-life care for all, Horizon Hospice also conducts education for physicians-in-training and other professionals, as well as affiliated institutions and the general public.
