Mission: The mission of healthcore clinic, inc (formerly known as center for health and wellness, inc. ) is to provide quality family healthcare through improved access and a heightened and unrelenting focus on education and prevention.
Programs: The most visible services offered by the healthcore clinic, inc. (hcc) are its medical services. Hcc provides family practice medical services to the community including immunizations, well woman/child exams, physicals (work, sports, annual), mediation assistance, acute care and on-site lab services. Hcc specializes in the treatment and management of chronic illnesses, especially diabetes, hypertension and asthma. Medical services are available 7am-8pm monday through thursday, and 7am-6pm friday and saturday. Providers are on-call evenings and weekends. Services are provided by physicians, mid-levels, nurses and medical assistants. Students from local colleges also complete rotations throughout the year at hcc. Most forms of insurance are accepted. Hcc also utilizes a sliding fee scale to determine fee for service amounts for uninsured and underinsured patients who provide proof of income less than 200% of the current, federal, poverty guidelines.
the healthcore clinic, inc's court approved and state licensed behavioral health counseling program is a confidential outpatient program that is intensive and comprehensive, utilizing rehabilitation therapy for people with mental health and/or drug and alcohol problems. The counseling program offers individualized treatment plans, including goals and objectives, progress reports to referral agencies, one-on-one consultations, one-on-one therapy sessions, family therapy sessions, group substance abuse counseling, random urine drug screens and aftercare groups for those completing group substance abuse treatment. Adolescent and adult anger management sessions are also offered by the counseling program. Included in the behavioral health counseling program is the strengthening families program (sfp). Sfp is designed to work with families that include children in the 10-14 age range, to help individuals in the families communicate and set family goals. The lessons taught in the seven week sfp sessions assist in helping this high risk, youth (ages 10-14) from turning to alcohol, drugs and gang activity.
the healthcore clinic, inc. (hcc) focuses on preventative health care over the traditional treatment model. It has dedicated programs in diabetes and hypertension care and self-management that incorporates nutrition education and physical activity. The outreach staff is in the community regularly at health fairs, churches and businesses providing preventative health-care education. Once a month, on-site screening is offered at no cost to the public. During the screening, an individual may get their blood pressure, cholesterol, glucose and kidney micro albumin levels checked; followed by consultation with a nurse about the test results. The outreach staff work with the medical staff in coordination of care provided to patients presenting with diabetes and hypertension. In addition, hcc employs two navigators who assist consumers in enrolling in the affordable care act health insurance marketplace.
