Programs: The patient navigator program works in partnership with the client to coordinate healthcare, remove barriers to care, and educate on self-managed care. The patient navigator is a healthcare professional trained on navigation and patient empowerment models to assist the client in getting the care they need when they need it. The patient navigator program also works with a volunteer medical transport coalition. In 2017, the program served 250 unique clients.
the health coverage guide program, in partnership with the northwest colorado visiting nurses association, provides free, in-person assistance and education for individuals and families enrolling in the state health insurance marketplace. In 2017, the connect for health data indicated that 1021 grand and jackson county residents enrolled in the marketplace.
the healthy grand county 2020 plan, created by healthcare providers in collaboration with community partners, is a blueprint to create a healthier community in 10 years. This plan focuses on access to care by assuring everyone has a primary care provider or a medical home, and on health education and literacy with an emphasis on patient responsibility and empowerment. Initiatives that focus on systemic changes, such as the meeting milestones initiative, fall under this program. The network is also the administrator of the healthcare professionals society, an all-inclusive society of healthcare professonals from all sectors who collaborate to "put the patient first", and the health and human services coalition, an all- inclusive coalition of non-profit and government agencies in the human services field working together to create long-term sustainable solutions and implement community-wide programs.
the mental health navigator (mhn) program is a collaborative between grand county government, mind springs health, and middle park medical center. Master's level clinicians provide brief intervention to improve access to behavioral healthcare in primary care provider and other community settings, educate patients on the importance of mental health services, and navigate patients to a long-term provider. The mhn focuses on behavioral health coaching (e. G. Brief intervention), removing barriers to care (such as stigma and insurance status), navigating to long-term behavioral healthcare, educating and empowering the patients on behavioral health intervention and screening, and advocating for and coordinating patient care with pcp's and behavioral health providers. Additionally, the mhn program will also offer lay transportation navigators to transport patients in emergency mental health situations to in-patient hospitals and substance abuse rehabilitation throughout the state. 147,969 the partners for adults in need of services (p. S. ) is a program established by the network and its partners to provide healthcare vouchers for acute, non-emergent medical and mental health services to uninsured adults who qualify financially. In 2017, the p. A. I. N. S. Program served 12 unique clients. 41,545 advocacy for children's health and education services (a. C. H. E. S. ) is a collaborative effort of grand county agencies and community groups to promote the well being of children. The aches program provides medical, dental and mental health vouchers to uninsured children who qualify financially and are in need of acute, non-emergent care or preventative care. 18,212
