Mission: Nearly 25 years ago, Gateway (GCC) was founded on the principle that all individuals including those with intellectual and developmental disabilities deserve to live happy, productive, independent lives. Since we first opened our doors, we've served more than a thousand clients by pioneering innovative programs that speak to them not only as individuals, but as shareholders in the larger community around them. All of our groups from a wide range of Areas of Interest are designed for people across the entire spectrum of disability. We pride ourselves on our caring, committed staff from our dedicated MSCs to our incredibly talented guest lecturers to our conscientious facilities-management team. Gateway programs run 365 days a year, 7 days a week, from early morning on into the evening hours, and we serve the Bronx, Manhattan, Queens, and Brooklyn. Our facilities are regulated under the auspices of the New York State Office for Persons with Developmental Disabilities (OPWDD) and the New York State Office of Mental Health (OMH).
Target demographics: Adults with developmental disabilities as well as those with severe and persistent mental illness
Geographic areas served: New York City
Programs: Gateway's Group Day Habilitation program features a wide-ranging menu of daily opportunities that are not otherwise available in traditionally designed programs. People with disabilities who attend our group day habilitation program work hand-in-hand with gcc staff to create a personalized schedule that helps them achieve their own unique goals - whether those goals are social, behavioral, or educational. In fiscal year 2013-2014, the enrollment reached 308.
Gateway's Supplemental Group Day Habilitation program provides individuals with an opportunity for structured activities that are more recreational and social in nature. Through the supplemental program, gateway attendees are able to further develop their own social skills while participating in a diverse range of fun, engaging group activities - and to create satisfying, meaningful relationships along the way. In fiscal year 2013-2014, total participants were 272.
Gateway's Continuing Day Treatment program (cdtp) provides a full therapeutic and social program for adults struggling with the often overwhelming challenges of living and coping with psychiatric illness. Licensed by the nys omh, the program includes various skill-building activities such as discussion groups, arts and crafts, budget management, and recovery. Expertly staffed by psychiatrists, social workers, and counselors, gcc omh's cdtp is characterized by its team approach toward treating those in its care. In fiscal year 2013-2014, total enrollment was 175 individuals.
Gateway Counseling Center's Community Habilitation program is designed for individuals who would benefit from more personalized attention. Individuals participating in the program are assigned a specific staff member who is trained to support and help specific issues that may prohibit an individual from fully realizing his or her full potential. The goal of the program is to support individuals in developing skills that will allow independence (in the home or the community). Goals can be as fundamental as bed making and brushing their teeth, or as complex as successfully navigating the community and interacting with strangers appropriately.
The Gateway Day Habilitation Without Walls program is born from the idea that Gateway can continue to support and nurture individuals we serve while being active members of the community. The Without Walls program stresses the importance of using natural supports in the community versus becoming overly reliant on staff members. The program has partnerships with several non-profits and local businesses that provide valuable volunteering experiences. The goal of the Without Walls program is to work towards maximizing the potential of the individuals that we serve. Based on the individualized goals for each person, Gateway creates a program structured to meet their needs and interests while continuing to increase independence and working towards actualizing their goals.
Gateway's ADVANCE program understands that every family, at one time or another, finds itself in the midst of a crisis -- a situation that can be particularly challenging for the families of those with intellectual/developmental disabilities. Gateway's Advance Family Support Services program offers compassionate, family-oriented guidance to help your family through this trying time.
Our caring team of professionals will work with all members of the family to develop an appropriate course of action -- with an emphasis on both behavioral modification and coaching parents and other family members in the skills they need to get through the current crisis. Gateway's Advance program also provides more long-term support by offering assistance in navigating those evaluative, diagnostic, or habilitative services that might be appropriate for the particular situation.
Gateway's Medicaid Service Coordination (MSC) program is program offering support to individuals and their families. Medicaid Service Coordinators begin to assist individuals in applying for and maintaining Medicaid, facilitating appropriate evaluations, filing and maintaining guardianship papers, as well as applying for recreational and habilitative services. Once these services have been established, MSC's monitor the services and ensure that the desires and needs of their individuals are met by working in collaboration with the individual and her or her advocate. MSC's develop an Individual Service Plan (ISP) and continue to review and modify services on an ongoing basis.
Individuals with intellectual and/or developmental disability who live at home or in group residences are eligible for Gateway's Medicaid Service Coordination program.
