Causes: Community Clinics, Health
Mission: Erie family health center's (erie) mission is: "motivated by the belief that healthcare is a human right, we provide high quality, affordable care to support healthier people, families, and communities. " meanwhile, erie's vision of "all people living their healthiest lives" demands that erie examine the social determinants of health and structural barriers that are the root causes of inequity in the communities it serves and works to address those challenges daily.
Programs: Erie family health center (erie) is a regional healthcare resource to more than 72,000 patients across seven large primary care centers, the region's oldest and largest comprehensive teen center, and five school-based health centers. To empower our mission and vision and to fulfill erie's values of dignity, impact, excellence, relationships, and learning, erie provides a comprehensive array of health services at one convenient location to empower lifelong wellness among the community's most vulnerable members. Since growing from a one-room location in 1957 to serving over 72,000 patients across 13 health centers in chicagoland, erie's commitment to helping meet the needs of underserved residents has remained steadfast; 90% of current patients are people of color and 29% are uninsured. Erie has been deliberate in its efforts to expand its locations into dynamic health centers that are uniquely positioned to meet the needs of community members. Erie health centers offer a wide range of high quality, affordable services, including integrated medical, oral, and behavioral healthcare; chronic disease management; vaccines and flu shots; hiv testing; sti testing/treatment; prenatal care; and women's health services, with programs intentionally designed to meet the needs of erie's individual patients. Erie services provide culturally-sensitive, trauma-informed care for patients, with providers and front-line staff who are bilingual in spanish and english, while offering translation services for those who prefer other languages. Erie will continue to invest in efforts to ensure staff are reflective of those it serves, which will, in turn, provide economic stimulus to the communities erie serves. Erie continues to strive toward achieving the high-quality outcomes that have placed it in the top tier of health centers nationally, with the u. S. Health resources and services administration recognizing erie as a "health center quality leader" every year from 2014 to 2017. Further, erie has been named a top chicago workplace, an award given by the chicago tribune based on erie employee feedback. According to the national uniform data system (2016), which tracks performance of federally qualified health centers like erie, erie ranks in the top 6% of health centers nationwide in combined rates of cervical and colorectal cancer screening. Erie also ranks among the top 8% for cardiovascular care. Further, erie delivers more babies than any other community health center in illinois and ranks in the top 1% nationally for number of deliveries.
