Causes: Centers to Support the Independence of Specific Populations, Human Services
Mission: Dssw is dedicated to helping people with disabilities reach their highest level of independence by providing quality and cost effective support services. We are the bridge that enables individuals to stay in their home and with their family and friends. Our care team is as diverse as those we serve. We are united by compassion and are driven by our responsibility to the community.
Programs: Class - community living assistance and support services: this program assists the disabled in achieving community integration. Working in coordination with a direct service agency the program seeks to offer personal care assistance, therapies, adaptive aids, and home modifications.
cds - consumer directed services: this program allows recipients of eligible services to manage the delivery of those services including attendant care. The program gives participants more flexibility in their schedules and greater freedom to select attendants and structure their pay.
dbmd - deaf-blind with multiple disabilities: medicaid waiver program which coordinates services with the appropriate direct service agency to offer personalized assistance, therapies, adaptive aids and home modifications.
phc/other programs - primary home care: medicaid waiver program which offers personal care and home assistance.
