Mission: The primary exempt purpose of the agency is to provide case management services to persons with developmental disabilities.
Programs: The agency provides case management through the medicaid service coordination program to individuals who have developmental disabilities and are eligible for opwdd services and medicaid. The agency assists the individuals we work with to live as independently as possible and to have as many choices in their lives as possible.
self directed services (sds): sds is a completely person centered program where individuals work with their team (or "circle of support") to develop an individualized and all encompassing program of supports. Each participant's program is unique to the individual for which it is created. Community living & advocacy supports, inc. Provides brokerage services to assist with the development and maintenance of these plans.
fiscal imtermediary (fi) providers are the "employer of record" for individuals receiving self-directed services. This is a medicaid funded waiver service. We support participants who self direct an individualized budget by billing and payment of approved goods and services, fiscal accounting and reporting, medicaid and corporate compliance, and general administrative supports.
