Mission: Pursuing healthier lives for the people of the sauk valley.
Programs: Equipment, supplies, and educational materials were purchased to support vision screenings and eye glasses for 129 children and 8 adults. The love on a leash program visited 1,200 patients. Provided 1,328 cholesterol/blood pressure/glucose screenings through cardiovascular wellness. Six reusable glucose monitors were purchased for the diabetes center. Workplace wellness assisted 611 patients with cholesterol/blood pressure/glucose/skin cancer screenings, classes and programs. Care notes provided for all waiting rooms. Funded 156 women though women's cancer. Educated 1,600 people through nutrition and fitness programs. Helped 165 patients through medication assistance program. Monitored 130 patients through community health network. These community projects served approximately 11,800 people.
assistance was provided to whiteside county health department to provide dental supplies to underprivileged children and adults. 3,800 children and 2,590 adults received care with this assistance.
scholarship assistance was provided to 54 individuals. Thirty-eight students received the delange scholarship, two nursing scholarship were awarded, and fourteen scholarships were awarded to students in the healthcare field.
