Programs: The homeless outreach and stabilization team (host) serves the formerly homeless adults in the northern alameda county. The goal of the program is to provide assistance to clients to: improve housing stability, increase employment and education opportunities, and to reduce hospitalizations and incarcerations. The host team maintains low caseloads with many services offered in the field where homeless people congregate. This allows the team to do "whatever it takes" to assist clients on their road to housing and recovery.
the supported independent living (sil) program currently serves approximately 150 clients. It offers a comprehensive range of services through three distinct components:1. The transitional supported housing program - serves as a bridge between highly structured residential treatment programs and independent living. 2. The permanent supported housing program - provides long-term homes and treatment for adults who are living with a severe and persistent psychiatric and substance use disorder and at risk of homelessness. 3. The case management program - offers assessment and evaluation, plan development, placement services, emergency intervention, advocacy, and assistance in daily living skills.
the dual treatment residential treatment is uniquely designed to treat adults living with a severe and persistent psychiatric and substance use disorder. The staff in this program is cross-trained in both mental health and substance use intervention and recovery. This program provides 24-hour services for up to 15 residents at a time.
