Mission: Better health together is organized and operated to, without limitation, promote the health of its region's residents through collaborative action around the triple aim (better health, better care, reduced cost).
Programs: Chronic disease self-management:discontinued at the end of 2016. Served as a community lead organization to coordinate the healthy communities obesity, diabetes, heart disease and stroke prevention efforts. This effort was funded through a contract with the washington state department of health.
navigator:as a lead agency, assists people in signing up for qualified health plans or medicaid through the washington health benefit exchange (hbe). Since its inception, this program has assisted over 130,000 individuals to enroll and resulted in 97% of people in the region having access to health care coverage. This effort receives funding from the hbe.
health, housing and homelessness (h3, includes hot spotters project):discontinued at the end of 2016. Hot spotters engaged high needs patients who presented to emergency rooms and emergency responders with a complex blend of social, medical, mental health and/or substance abuse issues. This program aimed to improve health outcomes for these medically vulnverable individuals through an innovative pairing of a community health worker and a housing specialist. This effort received funding from the city of spokane and the health sciences and service administration.
