When my child’s school requested he get up to date with his childhood vaccine, I immediately emailed National Vaccine Info Center to find a religious exempt form for the state of IL. I was surprised how quickly they responded to me with the link of where I could find the form. They really eased my stress and I was able to submit the form.

I’m thankful for organizations like this one.
