Mission: The organization was formed to provide home health care and hospice care to the people of southwestern connecticut. Services included; nursing, physical, occupational, & speech therapy, medical social services, and home health aides, along with hospice services which included palliative care and pastoral & bereavement counseling. Our mission was to provide exceptional home health and hospice services that enhance comfort, promote healing and independence, and improve the quality of life for the patients and families we serve.
Programs: There were no program services provided during the fiscal year ended september 30, 2017, as the organization ceased to provide home health & hospice services, effective november 30, 2011.
