Programs: Touchs eating well nutrition program is a comprehensive nutrition program based on the medical nutrition therapy (mnt) model. Mnt is a therapeutic approach to treating medical conditions and their associated symptoms via the use of a specifically tailored diet devised and monitored. The touch program staff and registered dietitian work with our members to improve health outcomes through improved nutrition education and by supplying access to healthier food options.
touchs medical case management program works with our members living with a chronic illness to address the many unique life issues faced by people living with chronic illness. This may include helping members arrange doctor appointments, getting medical insurance as well as assisting in obtaining help with legal, housing and general overall benefit assistance.
