Mission: The Washington Home & Community Hospices mission is to offer excellent healthcare and hospice services with great consideration and innovation, so as to encourage independence and dignity to its community members.
Target demographics: Those in need of hospice care, those who are chronically ill, those in need of long term and short term services, the elderly ill, those in need of subacute rehabilitation, those who have Alzheimer's, and those who have dementia
Geographic areas served: DC Metro Area
Programs: Long-term care - provide healthcare services for people who are aging or chronically ill. In fy 2014, 455 patients were serviced at the 192 bed nursing facility alzheimer's, long term, and rehabilitation care.
end of life care - provide hospice services for people who are living with terminal illnesses. In fy 2014, community hospices assisted 1,118 terminally ill patients with pain relief, symptom management, emotional support, and spiritual guidance.
