Guided by jewish values, montefiore is dedicated to a standard of excellence and personalized care, enabling individuals to live with dignity and security and as independently as they are able.
Programs: Nursing home care: the montefiore home provides nursing home care and therapy for the aged and ill. Montefiore strives to be recognized as the center of excellence for promotion of wellness treatment and care of illness, and attainment of the highest possible quality of life for the aging community.
homecare: from skilled nursing to physical therapy, montefiore's homecare professionals provide first rate healthcare services. Montefiore delivers compassionate, individualized healthcare right to the community. Montefiore's homecare specialists work with a patient's schedule to meet all their healthcare needs. Our homecare nurses and therapists act as case managers, coordinating with the physician and medical team to implement an individualized care plans for each patient.
hospice: montefiore hospice seeks to bring pain management and symptom control, as well as ultimate caring and compassion to those with a life-limiting illness, their caregivers, families and loved ones. We provide our signature care in the patient's own home, our inpatient unit or in other heathcare facilities.
