Mission: The health care improvement foundation is an independent, nonprofit organization that drives high-value health care through stakeholder collaboration and targeted quality improvement initiatives.
Programs: Patient safety - to assist hospitals in the implementation of evidence-based patient safety and quality practices, health care improvement foundation developed and led a collaborative in southeastern pa in urologic care for prostate cancer patients. Continuing with the 11th annual delaware valley patient safety awards program, regional hospitals' best practices for patient safety were shared and awarded. Health literacy - in collaboration with the thomas jefferson university and hospital, and other community partners, health communications are being developed and shared statewide through outreach and trainings. The philly difference - a collaborative funded by the city of philadelphia department of public health engaging physician practices to reduce patient hypertension and diabetes.
