Mission: Promoting and improving the general health and welfare of senior adults in buncombe county by administering a variety of programs and services designed to meet their needs.
Programs: Care management: providing assessment, advocacy, linkage and follow for identified services provided to persons transitioning from hospital or nursing homes and those receiving in-home aide services through the council on aging along with those identified as at-risk and vulnerable for community living. 135 people were allocated 41,119 units through in-home aide.
resource coordination (rc): includes outreach, education, and public awareness about aging services and needs while also providing advocacy, linkage and follow-ups. The rc program provided over 3,100 services to older adults.
seniors safe at home (ssah): a variety of services helping older adults maintain their independence and remain safely at home as long as possible. The ssah program provided more than 5,000 services to people in the community. More specifically 451 minor home repair projects were completed, and medicare staff and volunteers provided more than 4,400 educational sessions and appointments, and 1,162 affordable care act services were provided.
health and wellness: dining sites across buncombe county provide nutritious noon meals, socialization opportunities, and exercise classes in a variety of settings.
