Causes: Community Clinics, Health, Reproductive Health Care, Reproductive Rights
Mission: To improve the health of our patients and the community and to advocate the feminist goals of social, political and economic equality.
Programs: In fiscal year 2017, the santa cruz community health centers served 10,739 patients via 51,673 visits. We served nearly 800 patients who were homeless. This year we were also pleased to achieve patient-centered medical home certification, level 3, at both our clinic sites, a testament to our commitment to access and quality. As we continued to focus on complex patients with mental health, substance use, and homelessness, we invested in our integrated behavioral health program and care coordination. Our case management department grew with case managers providing intensive care coordination services for both adults and children and we worked closely with our local hospital, county mental health, and medi-cal managed care plan to improve care and reduce costs for patients who avoidably utilized acute care services. On the prevention side, we have articulated a vision for a pediatric center of excellence utilizing our experienced pediatric team to address the needs of low-income children and their families. Our partnership with the live oak school district and the live oak cradle to career program is improving collaboration with our education partners, particularly for high risk children with developmental and behavioral issues. As we head into 2018, we will be implementing team based care and providing additional training, mindfulness-based resources, and other support to staff for the challenges they face in serving patients with complex needs.
