Causes: Health, Health Support
Mission: To support and spread the use of best practices among healthcare providers in san diego, to engage healthcare stakeholders in enhancing the healthcare system and to support, educate and engage patients in population health efforts, including healthy behaviors and accessing high quality healthcare.
Programs: San diego accountable community for health (sd ach): the sd ach is a multi-sector partnership focused on creating a system for health and wellness with a focus on creating equity in san diego. Accomplishments in 2017 included the creation of a robust operational structure for the sd ach including a stewardship group and 3 workgroups. By the end of the year, a regular meeting calendar had been established and all of the groups were meeting regularly. In the coming years, this work will produce strategies for collective action, data sharing and community engagement.
post acute care collaborative (pacc) the pacc is a collaboration of healthcare providers, skilled nursing facilities and other agencies aligned in working together towards better coordination and transitions of care. Together these partners are sharing best practices, reviewing current outcomes metrics, and picking targets for shared outcomes.
