Causes: Developmentally Disabled Centers, Disabilities, Group Homes, Human Services
Mission: We partner with people with disabilties and people with other challenges to build relationships and develop skills to live and work as valued members of their community through affordable housing development, vocational training,career development, behavioral health supports and personal care training.
Programs: Group homes: theraputic homes operated for people with disabilities in which professional staff and direct support professionals assist and train individuals with disabilities to perform daily routines. The individuals receive long term professional support and case management in the following areas: activities of daily living, behavior support budgeting, health care management, leisure time and community integration activities. The services provided in the group homes are approved and regulated bythe funding authority. 25 individuals are served 365 days per year
supportive living: provides wrap around supports for people with disabilities who have the ability to direct their services. The professional support staff train and assist the individuals with disabilities served through this service in the areas of activities of daily living, money management, health maintence and community integration. The activities provided to the individuals are approved and regulated by the funding authority. 13 individuals are served with an average contact of 15 hours per week. This program is considered a long term residential service.
community based supports: services that provide direct support and assistance for participants, in or out of the participant's residence, to achieve and maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community. Community-based supports are delivered one-on-one with a participant and include assistance with community-based activities and assistance to, as well as training and supervision of, individuals as they learn and perform the various tasks that are included in basic self-care, social skills, and activities of daily living.
supportive employment: we provide vocational training and career counseling for people with disabilities who are interested in working. Professional staff assist individuals in job seeking activities, train individuals on the job, and perform follow along monitoring to achieve successful job outcomes. We offer transition activities for students graduating from school to work. 14 individuals receive intensive job coaching and follow along services. 26 individuals have been placed in permanent jobs.
