Partnerships For Healthy Communities

Programs: Chronic disease prevention - serves individuals across the lifespan, addressing obesity prevention and control with community partners through community building, policy development in schools and municipalities related to healthy eating and active living. In 2011, partnerships received multi-year grants to support our work in chronic disease prevention. As part of our january 2012 merger with commerce city community enterprise corporation public charities, partnerships legacy in chronic disease prevention will continue for many years to come.

early childhood health - serves infants and babies under age 5, convening teams to address health and wellness in early childcare centers, prevent gaps in health care coverage and promoting oral health in babies and children.

community engagement - partnerships for healthy communities is dedicated to engage the community in healthy initiatives in all areas of health and wellness. A successful example of such engagement was the construction and inaugural year of the los valentes community garden. Among our other community engagement initiatives, we are most proud of building a community garden that was built and produced vegetables and fruits by the community, for the community. Thanks to the support of the tri county health department, we were able to construct a garden that will serve the community for many years to come.

other programs include youth engagement, translation services and other activities to promote community involvement in personal and community health and well-being. Also included in expenses for 2011 is the transfer of assets to commerce city community enterprise corporation public charities, where the programs of partnerships for health communities will continue to flourish once the merger between these two nonprofit organizations is complete in 2012.
