Mission: By building capacity and forming strategic partnerships to promote health, we are furthering our vision of giving every person the opportunity to lead a healthy life. From donor-advised funds to co-investment partnerships, we help develop targeted programs that achieve results and improve health.
Programs: Northwest health foundation fund ii (nwhf fund ii) manages the kaiser permanente community fund, a twelve-year, donor-advised sunset fund with a mission to improve the health of communities throughout the kaiser permanente northwest service region, with a specific focus on addressing the "upstream" factors that create or inhibit community health, also known as the social determinants of health. In 2017, nwhf continued to pay out grants through kpcf, but our focus was on evaluating the impact this fund has had on communities throughout our region.
northwest health foundation fund ii (nwhf fund ii) manages the oregon active schools fund, a collaboration between multiple donors to make every school in oregon an active school, making the connection that active kids do better in the classroom. Awards went to schools that are working to support their students and are priority/focus schools, have a high percentage of free/reduced lunch, and low third grade reading scores.
