Causes: Health, Health Support
Mission: Developing a regional network of care for northwest colorado to include oral health, behavioral health, health promotion, disease prevention, chronic disease management and acute care for the entire regional population. We are doing this by: - supporting greater linkage between clinical and community partners to improve health outcomes - sharing funding and grants with community partners for building additional service capacity and resources - sharing innovative projects and systems level transformation through community informed processes - leveraging resources (time, money, expertise) for all partners - providing educational & outreach opportunities
Programs: The organization achieves its mission through collaboration across five counties with community service partners, clinical providers, and the community in order to identify the gaps in service and create community wide efficiencies that promote well-being for all residents. Leveraging regional data and focusing on community-driven solutions, they help create ways to deliver resources efficiently where they are needed most. This effort reduces redundancies and ensures efficient use of resources. By engaging with a diverse group of service providers and community members, the inclusive process facilitates connection of individuals and families with the right resources at the right time because they believe everyone deserves the opportunity to realize their healthiest selves.
