Mission: The mission of neighborhood healthsource is to improve the health of our communities by providing quality health care services that are affordable and accessible.
Programs: Primary medical care:the scope of our primary medical services is focused on maintaining access to comprehensive primary and preventive healthcare in order to improve the health status of underserved and vulnerable populations. The primary project service area is north and northeast minneapolis with nw hennepin county and southern anoka county as secondary service areas. The target population includes low-income people, both uninsured and those on medicaid. Services will be provided at four freestanding clinics: heritage senior clinic and fremont clinic in north minneapolis, central avenue clinic and sheridan clinic in northeast minneapolis. On-site services include primary care, prevention, outreach, ob/gyn, chronic disease, interpreting, and assistance with health care plan enrollment. We provide referrals and care coordination for dental, mental health and substance abuse, hospital and specialty services through partnerships with other community organizations. Physicians, nurse practitioners, certified nurse-midwives, clinical and administrative support staff provide nhs services.
community outreach - reproductive health:family planning special projects (fpsp)-description: project focuses on increasing awareness of family planning information and availability of low cost family planning services. To achieve this, the program includes the following components: public information, outreach services, counseling, methods, referrals and follow-up and includes services for all men and women of reproductive ages. Hennepin county safer sex initiative-description: this is a research grant with hennepin county research, planning, and development department (rpd). The safer sex program is a clinic-based intervention intended to reduce teen pregnancy and stds and improve condom and birth control use among high-risk adolescents. It is delivered to participants individually in a clinic by a health educator or other trained professional. Seen on da streets (male reproductive health)-description: maternal child health family planning services (mchsp) is a program to improve male reproductive health through outreach, education, sti screening and access to healthcare. The primary target is high-risk youth and young adult males, 15-26 who are low-income and predominately racial/ethnic minorities. Obesity, diabetes & cancerwe can prevent diabetes (ways to enhance children's activity and nutrition)-description: we can program is a science-based, adaptable national education program designed to help prediabetic individuals gain skills to improve their diet and increase physical activity through group education classes, tools, resources and incentives. Child obesity & diabetes prevention-description: united way funding kicked off nhs obesity and diabetes program fit force in the community and our clinics in 2008 (85k per year). This program delivers a community-based, family-centered obesity and diabetes prevention/management initiative engaging residents in north and north east minneapolis in healthy lifestyle changes. The focus is to reach children ages birth through ten (11-18) with recreational services, involve parents and other caring adults, and engage in collaborations that increase and sustain positive, healthy behaviors in the community. Cancer prevention-description: this project addresses breast cancer screening disparities for african american women through education about screening, delivery of primary care, healthy lifestyle referrals and education, health insurance options, and community resources for breast health. Funded through american cancer society. Oral health-dental health project (delta dental) aims to improve rates of dental referrals and access to care; increase numbers of patients receiving immunizations and of children accessing primary health care, and; develop and expand our preventive clinic- and community-based health initiatives with children.
supportive referred services:oral health-education on site includes baby bottle tooth decay (bbtd) education during well-child visits. Other dental care is provided by referral to other community providers including indian health board, southside, children's dental services, etc. Mental health-while nhs provides mental health care services on site for most patients, mental health care services for medicare patients are currently available through off-site referral to northpoint health and wellness center located in north minneapolis.
