Mission: The Wellness Coalition facilitates a coordinated, community-wide system to improve the health and wellness of people with limited or no health insurance through collaboration, services, and education.
Target demographics: Uninsured and Underinsured Adults
Direct beneficiaries per year: 1,076 clients through Wellness Case Management; 831 with access to free medications; 492 assists with the Federal Insurance Marketplace; and 2,219 individual chronic disease self-management education sessions
Geographic areas served: Alabama's River Region (Montgomery, Autauga, Elmore, Lowndes, and Macon Counties)
Programs: Wellness case management services, including linkage to a medical home for affordable primary care services, access to free medications and other community resources, and chronic disease self-management education to assist members of the target population to manage chronic diseases such as diabetes, heart disease, asthma, and obesity. Wellness classes, including the Chronic Disease Self-Management Program, Diabetes Self-Management Program, Mental Health First Aid, and the Change Your Lifestyle. Change Your Life. Diabetes Prevention Program. Continuing education opportunities for health care professionals to learn about prevention and management of chronic diseases and to earn continuing education credits.
