Programs: Case management: assists consumers on traumatic brain injury (tbi) waivers and community alternative for disabled individuals (cadi) waivers, and provides relocation service coordination within contracted counties and offers flexible case management services for consumers utilizing consumer directed community supports (cdcs). resource facilitation: information and resources program offers on-going support after hospital discharge for persons with brain injury and their family members, and offers resources and referrals to appropriate services.
