Mission: The lakewood resource & referral center's (lrrc) mission is to provide comprehensive, accurate and empathetic social services, information, referral, assistance, and advocacy, for government entitlement and private sector programs. Established in march 2005, the lrrc was created to help new jersey residents access, navigate, and understand the complex and often confusing government entitlement offices, information sources and applications. The lrrc assists residents in addressing the barriers they face as they attempt to navigate the social service and health care systems. We serve as a "one-stop" social services community agency by providing information about the many medical assistance and social service programs available to low-income residents, explaining guidelines and screening residents for eligibility, assisting residents with the application process, and providing advocacy and intervention services to assist lakewood residents in resolving problems with benefits and services.
Programs: In 2016, the lrrc assisted 8,172 unique households, reaching over 25,800 encounters, reaching over 40,000 individuals. The lrrc's services include assistance with affordable and fair housing, new jersey family care and other healthcare plans,snap and utilities bill/equipment assistance. The lrrc focused on providing all necessary tools to individuals to help educate them and enable them to become self sufficient. In this light, brochures were created and/or updated with new information on more than 10 social service entitlements. The lrrc maintained its line of collaboration with the local board of social services so that people in true emergencies will have someone advocate on their behalf. The lrrc also continuedits early detection program for breast and cervical cancer in conjunction with the susan g. Komen foundation. In 2016, the lrrc facilitated 110 mammogram screenings as a result of this initiative.
the center for health, education, medicine and dentistry (chemed) serviced over 24,000 unique patients in almost 125,000 face to face encounters in 2016, which was a 9% increase from 2015. The encounters occurred in the internal medicine, pediatric, behavioral health, dental and specialty care departments. They also included the newly opened women's health, immediate care, radiology and podiatry departments, chemed's pharmacy continued its growth filling over 51,000 prescriptions in 2016. Chemed sees all patients regardless of ability to pay, including those on government assistance plans and the uninsured who pay as per an income based sliding fee scale. Chemed provides health education opportunities to the community as well and is looking to grow all services based on community needs.
