Mission: To provide quality, respectful, & integrated health care - medical, dental, & behavioral health - for those who are underserved and lack other access to health care in la plata county.
Programs: The organization offers integrated, health care services including medical and oral, for those who have no other access or insufficient access, including but not limited to persons who are uninsured, underinsured and/or homeless. The organization developed and utilized a donated care model using licensed medical professional volunteers to provide care. During the year the organization provided direct services to approximately 400 patients, with information and referral services to 150 more. The organization did hire one type of professional, two dentists on contract, as it was not feasible to meet this need strictly through volunteerism.
