Mission: Housecall providers is dedicated to providing quality home-centered medical care, integrating and coordinating the delivery of primary, palliative and hospice serivces for homebound seniors and persons with disabilities.
Programs: Provide quality in-home medical care, integrating and coordinating primary care and palliative care medical services for homebound seniors and persons with disabilities allowing them to experience healthful lives in their own homes. In the current year doctors and nurses met with 1,863 patients and made 11,481 visits.
the primary purpose of the hospice program is to provide comfort measures and support for patients facing life-limiting illness, working side by side with families and caregivers. Services included nursing, aid with personal care and spiritual counseling. 401 patients were cared for and 14,486 visits were made during the current year.
